Care Management

We assist in  setting up  services designed to assist members and their care givers to best manage medical conditions  with the multiple goals of:

• Improving member health status
• Improving members psychosocial status
• Enhancing coordination of care
• Eliminating the duplication of services
• Reducing the need for expensive hospital visits

Companionship

We add a unique personal touch often missing from in-home care services. While we do help you care for your loved one physically, we also help keep your loved one social, safe and engaged.

Medication Management

We offer Medication Management services which is more than performing the technical task of administering medication. We provide an iterative process that requires the utilization of nursing knowledge, skill and judgment to ensure safe medication use.

24- Hour Supervision

Our staff is available on call to provide guidance and consult to care givers.  We offer monthly Supervision by a nurse or care manager to ensure that member benefits from the Multidisciplinary team.

Nursing Oversight

We offer clinical monitoring to effectively address the distinct needs and challenges presented in adult Foster Care.
