KRONOS Adult Family Care / Adult Foster Care

Adult Family Care (AFC) is a MassHealth funded program designed for those who are unable to live on their own but want the peace of mind and freedom of living in a home environment. AFC aids an approved family member, friend, or qualified caregiver who is providing in-home supervision of someone who is chronically ill or has a disability. We are devoted to helping elderly people and those with disabilities receive the care they need from friends or family in a supportive family and familiar setting.

Benefits For Caregivers

  • Tax-free caregiver stipend up to USD 1,580/month
  • Caregiver support and health coaching
  • Home visits from an RN and Case Manager
  • Coordinating social services including referrals to Adult Day Health programs and respite care.

Program Eligibility

  • MassHealth Eligible
  • 16 years of age or older
  • Unable to live alone due to a medical, physical, cognitive, or mental condition
  • Must have a qualified caregiver living in the home
